I Cliffbrina Jan Barry formerly known as Cliffbrina Mabel Joy Samura of 11c Wilkinson Road, Docorty, Freetown in the Western Area of the Republic of Sierra Leone do hereby wholly renounce, relinquish and abandon the use of the names Cliffbrina Mabel Joy Samura and in place thereof I have assumed with full legal effect henceforth the names of Cliffbrina Jan Barry. I shall henceforth be known, called, identified and distinguished not by my former names of Clifbrina Mabel Joy Samura but by the (assumed) names of Cliffbrina Jan Barry.

I shall at all times from today in all records, deeds and writings, and in all proceedings, dealings and transactions, private as well as public and on all occasions use and sign the new names of Cliffbrina Jan Barry, and that all documents and records bearing my former names, Cliffbrina Mabel Joy Samura shall not in any manner be deemed invalidated by virtue of this deed.

I therefore expressly authorize and request all persons at all times from today to designate and address me by my new names of Cliffbrina Jan Barry.
